Default Grinding/coughing upon crank?

The problem seems to be increasing of late. Only when cranking on cold, sometimes the engine will seem to almost grind or cough... very shaky when I crank the car. It will do this splutter for about two seconds, and then be fine and drives off fine. AUIII.

Any ideas?
