Default Rear disc conversion

I know there are a few people who have done this but I can only find bits and pieces through the search function. Can anyone provide a list of the parts required to convert to rear discs on a WP/WQ? I plan on doing the conversion and I'll take photos to be able to post a step by step in the technical section. I'm also looking at upgrading the fronts so if anyone has suggestions as to what fits there are well.

Heard the EBC slotted discs are a good upgrade on the front.

Already done RDA slotted discs. Was thinking bigger diameter discs and larger calipers with more pad area. Given the Focus discs will fit on the rear I don't know if the focus has larger fronts that would also fit.

I was talking to a bloke on here about upgrading the calipers, and from all accounts its very expensive for little gain over putting slotted disc's and greenstuff pads. But that was only his opinion. His name is Zetec Dave. He also sells the stuff, might be worthwhile contacting him.
