Default has anyone had a power steering oil resovoir cap disappear?

popped the bonnet on my mums car today to fill up the window washer fluid, and the power steering oil cap off the VCT was completely gone. :

$23 bucks later from sinclair Ford, we had a new one. but i have no idea how long it has been missing, or if any crap has gotten into the powersteering :

I gather when the car was last serviced the guy checking the fluid most likley did not replace the cap
